You will be asked to enter in your First and Last Name, Date of Birth, E-mail and Time zone.
Once you create an account you will get a temporary password sent to your e-mail.
Use the temporary password and your email address to log in.
Request your records.
How to request records:
1. Once logged into Clearpath, select the Request Record Tab.
2. Select the Request Record Tab.
3. To request a record, select the Request Record button on the bottom right corner.
4. Select Record Custodian (Facility Location)
*If you do not see the facility name listed, please reach out to support@myclearpath.com
5. Select which Records to Request, Body Part, and Date Range.
6. Agree to the terms and conditions and submit.
*Once submitted, our facility will review and process. Once request is processed, if approved, you will see your images under the My Records Tab.
